Ways to Refer to Moffitt
Online
Providers and medical staff can send patients to Moffitt for care by filling out and submitting our Online Referral Form. No login is required. Upon submission of the form, the referral confirmation page will provide the fax number to send corresponding patient medical records. Following your web referral a Moffitt specialist will contact your patient to schedule an appointment, often on the same business day.
Our secure portal allows referring providers and staff to login and view the clinical components of your patients’ medical records. Register for and access your Moffitt Physician Portal Account today.

About the Referral Process
You may be asked to send medical records to Moffitt to ensure your patient is scheduled with the appropriate clinical program and to preschedule any testing that might be needed when your patient arrives. The general fax number for medical records is 813-449-8210.
Moffitt New Patient Appointment Specialists will collect insurance information, verify benefits and seek financial authorization. If a patient is uninsured or unable to pay for services, a Patient Financial Services representative will be assigned to that patient’s case. After your patients' appointments, you will receive a copy of the findings and recommendations. If you have set up a Moffitt Physician Portal Account, you will also have online access to medical reports.
