How to apply to SPARK

Fill out an electronic application.

The completed application will contain:

1. Fully completed and submitted application via the Survey Monkey Apply system. Partially completed applications will not be considered.

2. At least one letter (up to three) of recommendation from a person with whom you share an academic or professional relationship. You will be asked to provide the email address for your recommender(s) in the electronic application.

3. Most relevant official academic transcripts. If one full year of college has not been completed by the time you apply, submit your high school transcript as well. Electronic transcripts are preferred and should be sent to SPARK@moffitt.org. If you do not have the option to send them electronically, have it mailed to:

Unofficial transcripts will not be accepted. If you are also applying to the ENCOURAGE-Diversity program, we can use the same set of transcripts for both applications. Please let the program know if you intend to do this.

All application materials must be received between Jan. 2, 2024 and Feb. 13, 2024.
