Advance Care Planning

Financial

  • The Andrew McDonough Be+ Foundation (Up to the age of 21 years, assists with expenses directly attributable to your patient’s diagnosis, find application online)
  • Family Reach (Up to the age of 29 years and in active treatment, organization dedicated to eradicating the financial barriers that accompany a cancer diagnosis)
  • Cancer for College (can apply between Nov-Jan. and have to be full-time student)
  • Children’s Cancer Center (Tampa area college scholarships for young adults diagnosed with cancer before 21 or had a sibling pass away from cancer)

Ask your social worker about a clinic resource list for additional resources available based on your diagnosis.

Wish Granting Program

  • Children Dream Fund
  • Wish Upon a Wedding (grant for a wedding or vow renewal, eligible if 1 person in couple is diagnosed with a terminal illness -- can assist with venue, catering, photographer, cake officiant, DJ, florist and more)
