The Department of Pathology at the Moffitt Cancer Center offers superior, individualized pathology services. Our nationally and internationally recognized subspecialists are available for expert consultative services to referring pathologists in private practices and hospitals by nationally recognized subspecialty diagnosticians. An extensive menu of anatomic pathology consultation services including immunohistochemistry, flow cytometry, molecular diagnostics and image analysis are available. Moffitt Cancer Center subspecialties include Bone and Soft Tissue Pathology, Breast Pathology, Cytopathology, Dermatopathology, Endocrine Pathology, Forensic Pathology, General Surgical Pathology, Genitourinary Pathology, Gastrointestinal and Hepatic Pathology, Gynecologic Pathology, Head and Neck Pathology, Hematopathology, Neuropathology and Pulmonary Pathology. Consultations and second opinions are provided with each interpretation initially communicated by a phone call to the referring pathologist and/or by fax. A final report will be issued within 24-48 hours of specimen receipt unless additional studies are needed that impact diagnostic interpretation.

Specimen Submission

To access Expert Consultation Services, please complete the Pathology Consultation Request Form. All applicable fields in this document are required to be filled out. The ordering physician's signature is required for all requests.

Please use this checklist as a guide when sending material to Moffitt Medical Group for Expert Consultation Services. If the consultation request form is incomplete, the slides will not be reviewed until all required information is complete.

This request to order tests from Moffitt Medical Group certifies to Moffitt Medical Group that (1) the ordering physician has obtained written informed consent from the patient as required by applicable state or federal laws for each test ordered and (2) the ordering physician has authorization from the patient permitting Moffitt Medical Group to provide the service and report results for each test ordered to the ordering physician.

Note: This service is not available for Medicaid patients outside of the state of Florida.

Payments for Services

Client Billing:
Consultation services will be billed by Moffitt Medical Group to the requesting institution. The requesting institution will receive one bill for both professional and technical services.

Third-Party Billing:
Moffitt Medical Group will attempt to bill the patient's insurance and collect payment. If payment is denied by the patient's insurance, the referring institution/provider will be invoiced for the services and will be responsible for payment. For Medicare patients classified as a hospital inpatient or outpatient on the date of service, any charges must be billed to referring intuition.

ICD-10 Codes are required for billing. When ordering tests for which third-party reimbursement will be sought, order only tests that are medically necessary for the diagnosis and treatment of the patient.
