Screening Access Program
Moffitt's Screening Access Program is managed by the Financial Clearance Unit (FCU) and was created to increase access to no-cost lifesaving cancer screening for uninsured and underserved community members.
The program is supported through a combination of funds from Moffitt and grants from various organizations. The number of no-cost cancer screenings provided each year depends on the resources available for that year. Additionally, navigation services linking community members to community resources are an offering of the program for breast cancer and residents of Hillsborough County, with the goal of expanding navigation services to other cancer sites and catchment area counties in the future.
Community members must meet certain criteria to be eligible to participate in Moffitt’s Screening Access Program. Eligibility criteria are unique to each cancer site based on the national screening guidelines as established by the U.S. Preventive Services; however, there are some general eligibility criteria across the cancer sites that must be met to become eligible for the program including:
- being referred by a healthcare provider
- not having any form of health insurance
- meeting the income guideline (at or below 200% Federal Poverty Level [FPL])
- not being on a tourist visa (may be eligible if in the country more than six months with an expired visa)
- having a photo ID
- not eligible for any other program (e.g., Medicaid, Medicare or Hillsborough County Healthcare Plan [HCHCP])

Mammogram Breast Screening
Q: What are the eligibility criteria?
A: To qualify for the Mammogram Screening Access Program, a patient must be:
- 40-64 years of age
- Or 30-39 years of age with a strong family history of breast cancer
- Or 30-39 years of age with other breast cancer risk factor(s), such as hormones
- Meet all social and financial guidelines:
- No health insurance or be eligible for Medicaid/Medicare or Hillsborough County Healthcare Plan (HCHCP)
- Meet income guidelines (less than 200% Federal Poverty Level)
- Not be on a student or tourist visa
- Live in Hillsborough County
Upon screening the patient for eligibility, community providers may refer eligible patients to Moffitt by sending the referral and screening access request form to:

Lung Screening
Q: What are the eligibility criteria?
A: To qualify for the Low Dose CT Lung Screening Access Program, a patient must have the following risk factors:
- Smoking history
- And/or personal cancer history
- And/or strong family history of lung cancer (one or more first-degree relatives)
- And/or radon or occupational exposure
- And/or disease history (such as chronic obstructive pulmonary disease (COPD) or pulmonary fibrosis)
- Meet all social and financial guidelines:
- No health insurance or be eligible for Medicaid/Medicare or Hillsborough County Healthcare Plan (HCHCP)
- Meet income guidelines (less than 200% Federal Poverty Level)
- Not be on a student or tourist Visa
- Live in Hillsborough, Pasco, Pinellas, and Polk counties
- Meet all National Comprehensive Cancer Network (NCCN) Clinical Guidelines:
- Be asymptomatic with no hemoptysis, coughing up blood or unexplained weight loss
- Be 50 years of age or older
- Current or former smoker with a 20-pack year smoking history
- Pack year = total number of years smoked X number of packs smoked per day

Q: What is a Community Screening Day?
A: Community Screening Day is an initiative of Moffitt's Screening Access program and coordinated by COEE’s community navigator to address the transportation needs of community members and increase access to breast cancer screenings. Each year, four quarterly screening events are planned by the community navigator with community clinics and community members (up to six participants per event) to arrange transportation to and from Moffitt’s breast clinic to receive no-cost mammograms. Initial results are provided to participants on the same day. The community navigator follows the participant closely before, during, and after the screening to provide navigation services to allow for linkage to resources in the community, helping to further reduce barriers to care and treatment. Note: the Community Screening Day program is currently limited to Hillsborough County residents and plans are being made to expand into other counties.
