Your Patient Services Specialist will gladly answer any questions you may have about becoming a patient.

Your cancer diagnosis is our top priority and we want to support you with compassionate care every step of the way. That's why we are connecting every new patient with a cancer expert as soon as possible. We want to ensure that you have a streamlined and informative patient enrollment experience. The first step to scheduling your appointment is to contact the Patient Appointment Center to speak with a Patient Services Specialist.

You can contact the Patient Appointment Center and speak with a Patient Services Specialist by calling 1-888-663-3488 or completing new patient registration form. We are available from 7 a.m. to 6 p.m. Monday-Friday and 8 a.m. to noon Saturday.

Your Patient Services Specialist will gladly answer any questions you may have about becoming a patient. Your specialist will ask you about yourself, your diagnosis, collect your insurance information and request your medical records for our physician's review. Your insurance information will be sent to a patient financial coordinator for verification.

If there are questions regarding your insurance after the verification process, your specialist may ask that a patient financial coordinator contact you to address your specific questions and coverage information.

Whether you are a new or an established patient in Florida, Moffitt also offers virtual care appointments. These visits may be a covered benefit, depending on your insurance company and plan.

Your Patient Services Specialist will ensure a Scheduling Coordinator helps you schedule your appointment. At this time, the scheduling coordinator will ask you to sign in to the MyMoffitt patient portal to complete your medical questionnaire and submit your medical records online so that our physicians have complete information on your medical history for review prior to your appointment. Your Patient Services Specialist and Scheduling Coordinator will ensure a clinical coordinator or nurse from your established clinic contacts you prior to your appointment to offer appointment confirmation and address any specific needs ahead of your appointment.

Once your treatment plan is established, Moffitt will work diligently to schedule your appointments. For appointments more than 30 days out, we will schedule you for the next available appointment. You can find all of your upcoming appointments in the MyMoffitt patient portal.
