Workshop Request Form
Thank you for your interest. Our workshops are tailored to the needs of the community and are offered free of cost. Each workshop will last 11⁄2 to 2 hours and includes a power point presentation, a one-time registration form, a pre and post survey and an evaluation. The pre/post and evaluation are electronic, interactive and easy to use. Attendees will receive canvas totes, water bottles, and pedometers for their active participation.

We provide workshops primarily in English and Spanish, however, we are at times able to deliver presentations in other languages (e.g., Haitian Creole, Burmese, and Arabic) along with community interpreters. Please use this form to submit a request for a workshop.
