MEDICARE AND MEDICARE SUPPLEMENTS

Moffitt Cancer Center and Moffitt Medical Group accept Original Medicare and accept Medicare assignment.

All Medicare supplements (also known as Medigap policies) are accepted by Moffitt Cancer Center and Moffitt Medical Group except for Medicare "Select" supplements. Medicare Select supplements require the patient to receive services from a contracted, in-network provider for the supplement to pay its supplemental portion. Moffitt Cancer Center is only contracted with one Medicare Select supplement (Ameri-Plus Preferred Care). 

MEDICARE ADVANTAGE PLANS
(ALSO KNOWN AS MEDICARE REPLACEMENT PLANS)

 
AETNA
  • Moffitt Cancer Center and Moffitt Medical Group are contracted, participating providers for Aetna's Medicare HMO and PPO plans.

AVMED
  • Moffitt Cancer Center and Moffitt Medical Group are contracted, participating providers for AvMed's Medicare HMO plans.

BLUE CROSS BLUE SHIELD OF FLORIDA (FLORIDA BLUE)
  • Moffitt Cancer Center and Moffitt Medical Group are contracted, participating providers for Florida Blue's Medicare HMO and PPO plans. (Florida Blue uses the name BlueMedicare.)
  • Moffitt Cancer Center is not contracted or participating with "Advantage 65," which is a Medicare Select supplement. Patients with Advantage 65 can come to Moffitt with their Original Medicare primary insurance, but their Advantage 65 supplement will not pay its supplemental portion. Patients can call BCBS to switch to a traditional Medicare supplement. (BCBS uses the name CompCoverage.)

COVENTRY
  • Moffitt Cancer Center and Moffitt Medical Group are contracted participating providers for Coventry's Medicare HMO and PPO plans. (Coventry uses the names Advantra, Summit Health Plan, and Vista Health Plan.)

FLORIDA HEALTH CARE PLANS
  • Florida Health Care Plans is a Blue Cross Blue Shield of Florida Affiliate.  As such, Moffitt Cancer Center and Moffitt Medical Group are participating providers for Florida Health Care Plans Medicare HMO and PPO plans.

HEALTH FIRST
  • Moffitt Cancer Center is a tertiary referral center for Health First's Medicare HMO plan, and Health First must authorize all referrals to Moffitt.  Moffitt Medical Group is not contracted with Health First but can see patients with authorization from Health First.

HUMANA
  • Moffitt Cancer Center and Moffitt Medical Group are contracted, participating providers for Humana's Medicare HMO plan. (Humana uses the name Gold Plus.)  However, this HMO plan requires that all services be authorized by the patient's Primary Care Physician.  Many Humana Gold Plus patients have had an extremely difficult time getting their Primary Care Physician to authorize services at Moffitt.
  • Moffitt Cancer Center and Moffitt Medical Group are contracted, participating providers for Humana's Medicare PPO plan. (Humana uses the name ChoicePPO.)
  • Moffitt and Moffitt Medical Group will accept all patients with Humana's Medicare PFFS plan. (Humana uses the name Gold Choice.)
  • Neither the Moffitt Cancer Center nor Moffitt Medical Group is contracted with Humana's AdvantageCare Medicare HMO plan.

UNITEDHEALTHCARE
  • UnitedHealthcare has chosen to exclude Moffitt Cancer Center and Moffitt Medical Group from its network of participating providers for its Medicare HMO and PPO plans. (UnitedHealthcare uses the names Secure Horizons, Evercare, AARP Medicare Complete.)  Patients with one of these plans can receive out-of-network services at Moffitt without authorization if their HMO or PPO plan has out-of-network benefits.
  • The Moffitt Cancer Center and Moffitt Medical Group will accept all patients with UnitedHealthcare's Medicare PFFS plan.
