Where You Are:

The most important records to bring are a history of your diagnosis and summaries of your hospitalizations, office visits and radiation visits.

Please contact your primary, referring, and any specialist physicians for this information, and ask them to provide copies for you to bring with you on your appointment day. Please include records of any chemotherapy treatments.

Be assured all information will be kept in the strictest of confidence, and no reports will be submitted to anyone without your permission.
