Becoming a Patient
Have you recently been diagnosed with cancer? Do you want to explore treatment options at Moffitt Cancer Center?
Whether you are a patient making your own referral or a physician making a patient referral, you can start that registration process below.
Patient Self-Referral
Option 1: Contact our New Patient Appointment Center at 813-745-3980 or 1-888-860-2778 (between 8 a.m. - 4:30 p.m. Monday through Friday excluding weekends and holidays).
Option 2: Online Pre-registration: Complete the New Patient Pre-Registration form.
- An intake specialist will call you within one working day to schedule an appointment with a Moffitt physician.
- She/he will verify insurance information and discuss how to handle your medical records, X-ray films and biopsy glass slides for your first appointment at Moffitt.
- After the appointment is scheduled, a New Patient packet will be mailed to each patient’s home address. The packet includes directions, contact phone numbers for the Moffitt physician(s), and a video about the Center.

Physician Referral
Option 1: Contact our New Patient Appointment Center at 813-745-3980 or 1-888-860-2778 (between 8 a.m. - 4:30 p.m. Monday through Friday excluding weekends and holidays).
Option 2: Online referral of a patient. If you are a physician or a physician-representative, complete the Referring Physician form.
- An intake specialist will call the patient within one working day to verify insurance and to schedule an appointment.
- The patient will be instructed how to handle medical records, X-ray films and biopsy glass slides for the appointment at Moffitt.
- The scheduling specialist may need to call your office for additional information.
